Complete Health Check-Up (Physicals)

The complete health check up of adults is the best time for your doctor to focus on preventive care and promote health. Following the recommendations from the Canadian Task Force On Preventive Health care (CTFPHC) and other sources, your doctor will provide you education and counseling, physical exam, lab testing, immunizations, etc.

Please fill out the following patient questionnaires forms and bring them with you at your next scheduled appointment. Please also bring all the medications that you are taking, including any OTC products or herbal products, as well as your yellow immunization card for update. Never forget to bring your health card.

• Complete health check up questionnaires for adults and adolescents
• Osteoporosis risk assessment questionnaires (for 50+ only)
• Fall assessment questionnaires (for elderly 65+ only)

The Ministry of Health has wisely initiated a number of preventive care programs and we advise you to take advantage of those that are recommended to you by your doctor.

• Colorectal Cancer screening which is a very simple stool test.  This is advised for patients between the ages of 50-75.
• Pap smears are advised for all women who have at anytime been sexually active and older than 21.
• Immunizations are essential in early life but many adults have fallen behind with their recommended ten year boosters.
• Mammograms are recommended for all women from age 50-69 as well as others outside this age range under specific circumstances.
